The UNIFIED review queue — everything awaiting human attention, tagged by `kind`: "catalog" = cataloger cards flagged low-confidence/unparseable (carry facets + confidence); "feed"/"relation"/"book" = things the agent ADDED (source:agent) not yet ratified — a feed, a relation edge, or a book holding (e.g. a to-buy suggestion). Each item carries the proposer `by` identity, a `rationale` (why), and `ratifyWith` — the tool to act on it: catalog → weir_reviewItem (confirm/correct), feed/relation/book → weir_ratify (bless or dismiss). Returns { counts:{catalog,feed,relation,book,total}, count, items }. Optional `kind` filters to one. This is the decides-vs-proposes gate: the agent proposes, you ratify here.
