Record how a question ended up being answered, so the page can show it in the history and save it to questions.jsonl. Use source "spoken" with the answer you distilled from the transcript when the user answered out loud, "typed" when you are copying a typed answer, and "skipped" (answer may be empty) when the user moved on without answering. Unknown question_id returns {error}.
