calculate_readiness

Compute the readiness score with its full breakdown: control coverage, verification, evidence freshness, exception health, and open proposals, each with its weight, sub-score, and the specific reason it is not 100%. The formula is returned with the result. The score is always recomputed from the live graph — approving a fix changes the graph, and the score follows; nothing adds points for activity. Do NOT report the score without its breakdown; a bare number is exactly the false confidence this system exists to prevent.
